Weight-loss surgery for morbidly obese patients will soon be available at Christchurch Hospital.

Bariatric surgery, which shrinks the stomach, was offered elsewhere in Christchurch for the first time last year, after the Government committed $8 million in 2010 to fund an extra 300 weight-loss operations.

Canterbury District Health Board (CDHB) planning and funding general manager Carolyn Gullery said the five South Island district health boards had pooled their elective funding to offer 42 surgeries to the most eligible patients, regardless of where they lived.

Canterbury had been allocated 13 surgeries in the 2011-12 year, 14 for 2012-13 and 14 for 2013-14. The surgery was available for severely obese patients aged 18 to 65, who would benefit the most from weight-loss surgery.

Gullery said previous operations performed in Christchurch had been done through a local private provider outside Christchurch, because of limited capacity caused by the February 2011 earthquake.

However, Christchurch’s hospital redevelopment plan would allow for bariatric surgeries to be done on site, she said.

The $600m plan was approved in September, and would involve building operating theatres at Christchurch Hospital, an expanded intensive care unit and emergency department, and a purpose-designed space for children.

Christchurch Hospital always had the capability to carry out the procedures in Christchurch, but the CDHB had not allocated them to it previously because the number of publicly funded operations available was very small, Gullery said.

“It was a better use of resource for those operations to be performed at Dunedin Hospital.”

The Health Ministry had not indicated if more funding would become available for bariatric surgery after the $8m was spent, Gullery said.

Dr Richard Flint, one of three surgeons carrying out bariatric surgery in Christchurch, said about 200 operations were done in the city each year, and popularity was growing.

The cost of the operation was the main obstacle to more people having the surgery, and Flint believed more operations needed to be funded publicly.

Matthew McConaughey has stopped losing weight for his new role after shedding 30 pounds (13 kilograms) in three months from his already lean frame.

The 43-year-old actor sparked concern after he recently dropped 30 pounds from his already lean frame for his role as an AIDS patient in the new movie ‘The Dallas Buyer’s Club’.

But a source close to the father-of-two – whose model wife Camilla Alves is pregnant with their third child – is now at his lowest weight.

The insider told People magazine: ‘Matthew starts shooting this weekend, so he’s at his absolute lowest weight. In a month, he’ll begin to put it back on.’

The actor has flown to New Orleans to begin filming the new movie on Sunday in which he plays Ron Woodruff, an AIDS patient diagnosed in the mid-1980s who seeks out experimental HIV treatment in a desperate bid to prolong his life.

Last August the ‘Lincoln Lawyer’ star admitted he found losing the extreme amount of weight to be a huge mental challenge but said he was determined to lose the 30 pounds required for the role.

He said: ‘I’m playing a guy who was sick and would have loved to have been healthier but wasn’t. For me, it’s more of a mental thing than a physical thing,’ he explained, adding that he’s been eating ‘small amounts’ of high-protein, low-carbohydrate food.

He added: ‘I’ll get down to the weight I need to get to. I’m on my way, and it’s what I need to do for the job.’

ARLINGTON HEIGHTS, Ill., Nov. 8, 2012 (GLOBE NEWSWIRE) — Body contouring after weight-loss (bariatric) surgery produces long-term gains in several aspects of quality of life, reports the November issue of Plastic and Reconstructive Surgery(R), the official medical journal of the American Society of Plastic Surgeons (ASPS).

“The study indicates a sustained quality of life improvement in post-bariatric patients after body contouring surgery,” concludes the study led by Dr. Eva S.J. van der Beek of University Medical Center Utrecht, the Netherlands. “This suggests the importance of including reconstructive surgery as a component in the multidisciplinary approach in the surgical treatment of morbid obesity,” Dr. van der Beek said.

Improved Quality of Life after Body Contouring Surgery
The researchers evaluated quality-of-life assessments in 33 patients who underwent body contouring after bariatric surgery. Body contouring refers to various plastic surgery procedures done to remove excess fat and skin after massive weight loss.

The most common procedures were abdominoplasty (“tummy tuck“) and operations on the breasts. Through an average of seven years after body contouring, the patients repeatedly completed a standard questionnaire evaluating obesity’s impact on quality of life.

The results showed “mostly moderate to large, sustained improvement” in quality of life in the years after body contouring. Scores improved in six out of seven quality-of-life domains, including physical functioning and appearance, mental well-being, social acceptance, intimacy and social network.

Some domains showed a small decrease between four and seven years’ follow-up. In general, quality-of-life scores were lower for patients who regained weight after their body contouring surgery.

Overall, 55 percent of patients were very satisfied with their results. All but one patient said they would undergo body contouring again, and considered it “an inevitable step to improve daily quality of life.” About one-fourth of patients had further body contouring surgery–another 30 percent said they would do so if their insurance covered it.

Bariatric surgery produces sustained weight loss in patients with severe obesity. However, more than two-thirds of patients with massive weight loss are left with loose or overhanging skin. This may lead to decreased satisfaction with the results of bariatric surgery, as well as psychological, social and physical problems.

Previous studies have shown that body contouring improves quality of life after bariatric surgery, but the long-term benefits are unclear. “There is an ongoing debate if body contouring surgery is an optional or essential step after massive weight loss in the treatment of morbid obesity,” Dr. van der Beek and coauthors write.

The new study shows significant and lasting improvements in quality of life for patients who have body contouring after bariatric surgery. “This suggests the importance of including reconstructive surgery as a component in the multidisciplinary approach in the surgical treatment of morbid obesity,” the researchers write. They call for further study of the long-term benefits of body contouring–including possible reasons for the decrease in quality-of-life scores a few years after surgery.

People with type 2 diabetes often are encouraged to lose weight to improve their health, but a puzzling new study may have some questioning that advice.

A recent study showed that obese people with diabetes who lost a modest amount of weight didn’t lower their risk of having a heart attack or stroke, but the weight loss did help improve many other health factors.

“While we did not show a benefit of modest weight loss for cardiovascular events, we did show the benefits of weight loss for improving depression, quality of life, sleep apnea, incontinence, fitness, physical function and blood sugar control,” says Rena Wing, a professor of psychiatry and human behavior at Brown University and chairman of the Look AHEAD (Action for Health in Diabetes) study. The research was financed by the National Institutes of Health.

In diabetes â?? a disease spotlighted in November during American Diabetes Month â?? the body does not make enough of the hormone insulin, or it doesn’t use it properly. Insulin helps glucose (sugar) get into cells, where it is used for energy. If there’s an insulin problem, sugar builds up in the blood, damaging nerves and blood vessels. There are two major forms: type 1 and type 2, which accounts for 90% to 95% of diabetes.

Diabetes can lead to heart disease, stroke, kidney failure, foot and leg amputations and blindness. Almost 26 million children and adults (8.3% of the population) in the United States have diabetes.

For the study, researchers at 16 centers around the country worked with 5,145 obese people with type 2 diabetes. Their average starting weight was about 200 pounds. The study was designed to see whether weight loss reduced the risk of heart attacks and strokes.

Half of the participants were assigned to an intensive diet and exercise program. They attended individual and group meetings where they were taught strategies for weight loss by cutting calories and increasing physical activity. They used some meal replacements during the weight-loss and maintenance phases of the program.

The other half received a general program of diabetes education several times a year. All participants continued to follow the medical advice of their physician.

Findings: Those in the diet group lost about 8% of their starting weight (about 16 pounds) in the first year and maintained nearly 5% (about 10 pounds) of that loss. Participants in the diabetes education group lost about 1% (2 pounds) of their starting weight and kept it off. Everyone was tracked for up to 11 years.

After four years, the findings showed that those in the diet-and-exercise group improved their hemoglobin A1C readings (which looks at a person’s average blood sugar levels for the past three months), fitness, systolic (top number) blood pressure and HDL (good) cholesterol more than those in the diabetes education group. The diet-and-exercise group also required less medication to control the diabetes, Wing says.

Participants in the diabetes education group, however, had better LDL (bad) cholesterol at year four, possibly because more of them were taking cholesterol-lowering drugs, she says.

There was no difference between the two groups in the incidence of heart attacks and strokes, but the incidence was low in both groups, Wing says. “The fact that they had lower rates of cardiovascular disease than we expected probably reflects improvements in medical management of diabetes.”

Physicians today are working harder to make sure the patient has good blood sugar control and has good blood pressure and cholesterol levels, she says. “There has been much more intensive medical management of people with diabetes to prevent heart disease.”

Wing says researchers will continue to monitor the study participants to see whether their weight loss lowered their heart attacks later in their lives and to assess other possible long-term benefits of weight loss.

“The study provides important information, but we don’t know from this study whether larger weight loss would have better impact, and we don’t know if these weight losses would have decreased the risk of heart attacks and strokes in people without diabetes,” she says.

James Hill, executive director of the Anschutz Health and Wellness Center at the University of Colorado and the principal investigator at the study’s site in Denver, says the bottom line is still: “Losing weight is one of the best things you can do for your health if you are overweight with type 2 diabetes.”

Thomas Wadden, director of the Center for Weight and Eating Disorders at the University of Pennsylvania’s Perelman School of Medicine, adds: “Regardless of its effects on heart disease, modest weight loss and increased physical activity improve multiple aspects of health and well-being in overweight individuals with and without diabetes. Let’s not throw out the baby with the bathwater.”

Sue Kirkman, senior vice president of medical affairs for the American Diabetes Association, says the message is that there are lots of other benefits for weight loss, such as needing fewer medications and better quality of life, and “we know that weight loss helps prevent or delay type 2 diabetes.”
